Summary learning points
Case # 1 from Chulalongkorn by Dr. Patita Sitticharoenchai

1. Young man with cyanosis with increased pulmonary blood flow
2. Agitated saline opacification sequence: RV then Ao (without entering LA LV )
This is very puzzling. But it means that there was a right to left shunt at the level of ascending aorta.
3. Large AP window can present in adult life !!




Case#2 From Ramathibodi Hospital by Dr. Teerapat Yingchoncharoen

1 Mildly impaired LV systolic function with moderate PH which was difficult to explain the overt signs and symptoms of left and right sided failure
2 High LVOT velocity was noted despite impaired LV function !!
3. AV fistula on the abdomen was eventually discovered to be the cause of high output failure.






Case#3 From Chiang Mai University by Dr. Rungsrit Kanjanavanit

18 y/o girl
Central cyanosis with Hx of repeated brain abscess.
1. Echo show large ASD bidirectional shunt. RV pressure load , TR PPG 97 mmHg
2. ASD Eisenmenger was originally diagnosed. Patient was advised against a corrective surgery.
3.CXR did not add up. (PA not particularly enlarged)
4. Careful segmental approach echo revealed transposition of great arteries
RV was the systemic ventricle hence the high TR Vmax.
5. d-TGA was diagnosed and an atrial switch operation is planned.
